When you come in there will be one brief consent form to sign in person. This makes the process faster and easier, saving you more time for your relaxing acunap when you get here. Who is this form for?

If you have NEVER been here for acupuncture OR it has been more than 5 years since your last time in you need to do this form either here online or at your appointment. This is not the covid protocols form. This is the intake form for new clients. NOTE: Do not print this form, if you prefer to do a written form we have that available in the clinic at your first appointment. This form is for online submission only.
